Dear Parent/Guardian:

Welcome to Beulah Elementary School! All students entering a Florida school must have the following:

•Certified Birth Certificate

•Florida Certification of Immunizations (blue form)

•Child's Social Security Card

•Physical Examination (must be within the current school year)

•Proof of Residence (one of the following):

1. Official Rent Receipt

2. Gulf Power Bill, ECUA Bill or City of Pensacola bill showing the parent/guardian's name and service address (not mailing address) **We do not accept phone or cable bills.

•If you are residing with someone, the person you are living with must provide their current Proof of Residence as stated above, a letter stating that you and your child/children reside with them at that address, and complete a Residence Documentation Form in the office. These three forms will establish your proof of residency.

•Final Report Card from former school.

•Valid Florida Driver's License
